Although this section can remain blank, for both guardians to access the FA Matchday App, full details of both parents/guardians need to be registered with the club and TheFA.

PAYMENT OPTIONS

Signing On shall be charged as a one off payment of £10. This payment is to be made via theFA Matchday App prior within 7 days.

The Annual fee (September - May) shall be charged either as a one-off payment of £135.00 (pro rata), or via x9 £15.00 monthly instalments (pro rata). Both methods to be paid via theFA Matchday App. The initial annual fee for monthly payments is due 1st September 2024.

DECLARATION

I give permission:

• For the named player within the online registration to attend training and to represent Lydney Town AFC Youth (LTAFCY) during the current season (inc. tournaments, friendlies and competitive matches).
• I give permission for appropriately trained LTAFCY officials to provide first aid to the named player in the event of illness or injury and give appropriate permission to healthcare professionals dealing with any such injury or illness. I understand that whilst LTAFCY coaches, officials and assistants will take all reasonable care and precautions to ensure the health and safety of players, they cannot be held responsible for any loss, damage or injury suffered whilst travelling to and from fixtures or whilst participating in matches or training sessions.

I, as the Parent/Guardian:

• have read and understood LTAFCY Club Rules, LTAFCY Code of Conduct for Parents/Guardians and LTAFCY Code of Conduct for Players. Both I and the named player within the online registration understand the implications of not adhering to the codes.
• certify that the information provided within the online registration form is correct. I agree for it to be used by LTAFCY for any purposes under LTYFC Privacy Policy. I understand making false or inaccurate statements or failing to disclose information will render the registration invalid and liable for sanction.
• accept that LTAFCY are not responsible for the children’s personal needs and parents should be present at all times (as applicable) for welfare reasons.

I understand:

• I must pay the initial registration fee via theFA Matchday App within 7 days of registering through LTAFCY website, and prior to playing in any league fixtures. If not paid, the club shall remove my player’s registration prior to the start of the season.
• A player whose annual registration is more than 2 (two) month in arrears and/or 3 (three) match fees in arrears shall be subject to a playing and training ban which shall only be lifted once full payment of outstanding amount is received unless alternative payment methods are agreed with the Club Treasurer and Secretary.
• A member who remains in debt from the previous season will not be able to register with the club until all outstanding debt has been cleared, unless otherwise arranged with Club Secretary and Treasurer.
• LTAFCY reserves the right to cancel membership registration without refund in the event that conduct, on or off the pitch is deemed unacceptable by either the player or the parent(s) by the LTAFCY Committee.
